Primary Care Network 1D (PCN1D) in West Barnet are looking to expand their team and are looking for a Digitaland Transformation Lead to support increasedaccess to care for patients, supporting the adoption of new technology andother initiatives to improve the care offer, and enabling PCN staff to workmore effectively to support the sustainability of general practice services.

Main duties of the job

Work within the PCN on a range of digital transformation projects such as the use of digital triage system i.e., e-consult, online signposting and promotion of the NHS App

Lead an improvement approach to change including building capability for quality improvement within the PCN and system wide approaches to problem solving

Providing operational management to PCN activity

Ensure that digital and operational PCN transformation is embedded in, and aligned with, ICS and national strategies.

Willingness to work flexibly, demonstrating problem solving skills and an ability to respond to sudden unexpected demands

About us

Barnet Federated GPs CIC is a not for profit organisation, putting GPs and healthcare professionals at the forefront of Primary Healthcare provision in Barnet. The Federation is building a large clinical team who will work as one in developing Primary Care Services in Barnet and will support Primary Care Networks.

The Federation was formed in 2015 and aims to improve the quality of health in Barnet through sustainable primary care. Its current membership consists of all 50 primary care practices within the borough. Although we are a private company, we hold a strong affiliation to the NHS, its constitution and guiding principles.

Produce reports and analysis which show attainment and progress in respect of project reports, data quality audits; interpret and analyse results and feedback to PCN Exec and member practices.

Work together with all local partners to increase the strengths and capacities of local communities, enabling local VCSE organisations and community groups and local residents to thrive.

Support the effective adoption of national and local initiatives, including integrated working at neighbourhood and place level to improve access to services for patients; and

Ensure that digital and operational PCN transformation is embedded in, and aligned with, ICS and national strategies.
