Salary: – £31,087 – £32,268 p.a. (37.5 hoursper week) (depending on experience)

We have been providing exceptional care to people living with a terminal illness in Maidstone, Aylesford, Tonbridge, Malling and surrounding areas since 1991. We support patients with both cancer and non-cancer diagnoses, and those closest to them, and all our services are provided free of charge.

Our vision is that everyone living with a terminal illness in our community will have the best quality of life, for as long as they live. Our highly trained colleagues and volunteers support our patients to live in comfort, with independence and dignity in their own homes, local care homes or in our Hospice. All our care is underpinned by our core values of compassion, integrity, respect and teamwork.

As a local charity independent of the NHS we are reliant on our supporters and donors, our services cost £5 million to run, with over £4 million raised through the generosity of our local community

Main duties of the job

Working as one of the hospice multidisciplinary specialist palliative care team in the provision of a co-ordinated and comprehensive service to patients, their families and carers. This will be achieved through the provision of specialist palliative care interventions based on a comprehensive patient centered assessment, agreement of goal focused care planning, education and support to patients, families and carers.

The Dementia Team support is provided to patients and carers in their own place of residence through the planning, implementation and evaluation of clinical care to promote independence, achieve the best possible quality of life and understand patients own likes, dislikes, and preferences.
