An exciting opportunity has arisen in our expanding team, for an enthusiastic and self-motivated individual to join the Track and Triage Team working in a hub and spoke model.The role of the RDF will be based in Queens Hospital Burton in the week with weekend working from the Great Wyrley hub, providing discharge guidance to our acute hospital trusts.The successful candidate will be part of a triage team that receive referrals for patients with complex needs who require ongoing support. The role includes the triaging of patients’ needs as part of a multi-disciplinary team with a key role in decision making. The post holder will liaise with a variety of people (service users, colleagues, member of the acute hospital team, families/carers and other professionals) to provide an accurate assessment of an individual’s ability/potential for reablement via the discharge to assess pathways alongside supporting admission avoidance assessments to prevent a hospital admission.The post holder will use their own initiative and professional judgement although they will be well supported by the surrounding team and through the clinical and professional networks accessible through the community teams.We are looking for someone that has a passion for making a difference to our patients, someone who wants to enable individuals to live their best lives through the use of our reablement service.

Main duties of the job

  • The post holder will be part of the South Track and Triage team responsible for liaising with both an acute hospital setting and a Multi-disciplinary community setting.
  • The post will be required to travel on exception to cover at all acute sites and to work at the triage base.
  • The post will be providing a 7 day a week service operating from 8am to 8pm, the post holder will be expected to work on a rota basis giving coverage across these hours 7 days a week
  • This is a dynamic post that requires the post holder to work closely with multi-disciplinary teams/agencies in addressing a range of care/support issues, preventing unnecessary hospital admissions, promoting independence and improving the health and wellbeing of the population served.
  • This post holder will manage complex referrals from a range of disciplines including Physicians, Social referrals and end of life care providers.
  • The post holder works without direct supervision and sometimes within the scope of practice and in accordance with their own competence.

  • The post holder will be responsible for working collaboratively with other Trust services to provide seamless care for adults/carers on defined care and support pathways with particular emphasis on facilitating the discharge of patents through the D2A model.
  • The post holder will promote and champion good practice in all social care provision including; working in partnership with agencies external to the Trust supporting adults/carers to access services/support with independent, non-statutory and voluntary sector providers.
  • The post holder will be supported by the Trust to develop their skills, knowledge and confidence in line with national requirements. This includes having a personal development plan; and having dedicated allocated time for personal development and regular formal supervision.
  • The service will be delivered in accordance with the principles of Recovery; empowering individuals to take control of their lives and putting them at the centre of everything we do, recognising the rights of all individuals to gain access to valued roles, relationships, activities and facilities within their local community.
  • The post holder will facilitate the patients safe transfer from hospital back into the community and to liaise between the hospital and other statutory and non-statutory agencies as required.
  • The post holder is expected to represent the organisation in a professional and courteous manner at all times and provide advice and information using a level of Social Care understanding and professional judgement.
