Weve made exciting progress with our virtual ward and patient numbers are continuing to increase in line with our plan to reach 250 patients by the end of the year. We already have a team of experienced clinicians supporting the virtual ward and we now wish to add pharmacists to our VW team to ensure they have access to medicines experts.
We are looking for someone who has extensive clinical pharmacy knowledge and experience, and the ability to work with a multidisciplinary team. You will need to have excellent communication, clinical and negotiation skills, as well as experience of managing staff, leading a team as well as providing education and training.
Main duties of the job
Inthis role you will be the lead pharmacist for the provision of comprehensiveclinical pharmacy services to the Virtual Wards as well as supporting ourprimary care network. You will be embedded in the multidisciplinary teamproviding care to respiratory, cardiology and frailty patients in the Virtual Wards.In addition, you will support further development of clinical andpharmaceutical pathways. Antibiotics will often play an important role for ourpathways, and you will be key in ensuring the antimicrobial stewardship isappropriately managed. You or your deputy will be required to attend theSystems Antimicrobial Stewardship Group as a pharmacy representative.
Thisis a patient facing role, so dont let the word virtual put off. This meansyou will visit patients in their own homes or in care homes, allowing you toprovide a full holistic review of the patient whilst gaining the additionalinformation around medicines concordance which only a home visit canprovide. with an expectation that youwill be prescribing or deprescribing medicines.
