Could you be the person we are looking for to help us to strengthen our Locality Coordination Centre (LCC) Team in Weston? As an exciting development we would like to implement a Planned Nursing Coordinator as a new permanent role within the Weston & Worle Locality.

We are looking for experienced nurses, with excellent triaging; communication; and delegation skills. This will be an office-based role, although there will be opportunity to attend clinical visits and maintain clinical competency.

Our LCC is the main hub of the locality where we receive and process referrals for each pathway (Planned Nursing; Urgent Care Response; Pathway 1; and Therapy). We have a planned Nursing Coordinator who manages and triages the incoming referrals and allocates the visits out to our team.

Our Integrated Network Teams (INTs) deliver out of hospital care to people in their own homes or community clinics. We provide these services to people who are registered with a GP within North Somerset. The INTs are aligned to Sirona Localities, and these, in turn, are aligned to groups of GP practices, known as Primary Care Networks.

Main duties of the job

In this role you will be working as the first point of contact for the locality, receiving phone calls and referrals from our Single Point of Access (SPA). You will then be responsible for the triage and delegation of these out to the team. We receive urgent 2 hour response referrals, same day referrals, and planned referrals.

We provide both planned and unplanned care during the hours of 8am 8pm, 7 days a week. We are looking for someone who is able to work Mon-Fri, within the hours of 8am-8pm (8am-4pm;12pm-8pm or 8am-5pm). While the majority of shifts would be Mon-Fri (which would include some Bank Holidays), there may also be occasional requirement to work at weekends to help provide 7 day cover for the service.

To maintain accurate, informative, legal and contemporaneous records of all interventions in accordance with Sirona care & health policy and The Code Nursing & Midwifery Council (NMC) principles and practice

Actively participate in training; this may involve assisting in the delivery of training both within the service and to other health professional as appropriate.
