Job summary

We are lookingfor a substantive Specialty Doctor in Geriatric Medicine to join our Ageing andHealth Department. There are currently 8 substantive Consultants (many lessthan full time), 3 Locum Consultants and 3 Associate Specialistsproviding senior leadership to frailty services across the organisation. Services include the Acute Frailty team,Frailty wards, Orthogeriatrics, Stroke, inpatient Older Peoples Advice andLiaison service and Proactive care based at Milford Hospital.

We welcome applicants with at least 4 years experience in general and Geriatric Medicine. We have developedlocal Tier 3 training to support career progression for Specialty Doctors andother allied healthcare professionals in Geriatric Medicine and would envisagepost-holders to may wish to remain in long-term Specialty Doctor posts, orrotate four monthly through different frailty services developing a portfolioof competences and progress to become an Associate Specialist or a Consultantthrough the CESR route applying for entry on the Specialist Register.

Thisis a post at Specialty Doctor level as per the SAS doctorscontract. It is likely to rotate everyfour months within Ageing and Health to ensure a wide range of experience. It currently has no weekend commitments butis likely to contribute to seven day working in the future with day-timeweekend duties (maximum 1 in 4).

Main duties of the job

Supportingthe Consultants in leading and supervising the team in the day to daymanagement of patients following a comprehensive geriatric assessment approach

Teammembers should attend a daily huddle or SAFER board round to ensureprioritization and efficient use of time

Workingclosely with the wider multidisciplinary team to ensure patients are managedeffectively and efficiently proactivelyliaising with patients families / next of kin (with patients permission wherethey have capacity or otherwise in their best interests) to ensure anunderstanding of frailty and management plan

Supportingjunior doctors in their training and completing assessments

Allmembers of the team are responsible for ensuring patient flow and effective bedusage by prioritizing discharge planning / completion of Electronic dischargeletters

Workingwith iOPAL consultant to provide initial frailty review for patients identifiedwith frailty on non-frailty wards
