To act as care coordinator for those service users settled in placements in south west Hertfordshire who require an annual review of their care .

Supervising a Band 4 STaR worker dedicated to providing support to those in Community placements

Main duties of the job

We are looking for a motivated and skilled mental health practitioner to join the Adult Community Mental Health services in a specialist post as a placement care coordinator for service users in funded social care and health placements for the South West quadrant of Hertfordshire.

You will need to have a good understanding of social care and the Care Act. The post holder will have the ability to assess need and commission packages of care to promote strength based interventions with a focus on the Recovery model.

The Post holder will work with a Support Time and Recovery worker as part of the Adult Community Mental health staff team and liaise closely with key staff members in the Continuing Care and Placement Service. You will need to coordinate annual placement reviews and respond as and when required to change in service user need. Additionally, you will develop and foster good relationships with placement providers and the Adult Community Mental Health Service and work alongside them to promote and deliver the Connected Lives Programme.
