An exciting opportunity has arisen within North Tees and Hartlepool NHS Foundation Trust to appoint inspirational and enthusiastic Band 5 Community Staff Nurse to join one of the community nursing teams caring for patients in their own or residential care homes in the localities of Hartlepool and Stockton. This maybe Rapid Response, District Nursing or Holdforth Community Hub and Homefirst . You will be joining a leading organisation that is both forward thinking and innovative. Main duties of the job

We are looking for motivated professionals with excellent communication skills who have the ability to work independently using a population health approach.
