An exciting opportunity has arisen for a motivated Young People’s Practitioner, Band 4 to join our 0-19 Children’s Service, which has been rated as ‘outstanding’ by CQC.

You will need good interpersonal skills to communicate effectively with young people, their parents/carers and a wide range of professionals.

You will be a skilled team player working under the guidance of the Specialist Community Public Health Nurse (SCPHN).

This role is in Derby City and you will be required to travel around to numerous schools, bases & to partner agencies.

Main duties of the job

To work as part of the team and under the guidance of the Band 6 Public Health School Nurse Lead to ensure delivery of the relevant touch points for example Reception, Year 6 and mid teen health assessments.

Support the School Nursing team in implementing care to address children and young people’s needs and offer age-appropriate information and advice in relation to physical, emotional and sexual health by using evidence based tools.

You will support children and young people in a variety of settings which includes schools, health centres and home visiting.

  • To work within the multidisciplinary team to provide an integrated approach when working with vulnerable families and contribute towards safeguarding plans under the direction of the School Nursing team.
  • Contribute to the planning of, and delivery of health promotion activities as directed by the School Nurse and public health data.
  • Complete appropriate referrals to other professionals/services for children/young people you have been involved in, under the guidance and approval of the qualified nurse.
  • To record all visits via SystmOne with 24 hours and ensure record keeping is contemporaneous and within Trust policy and to report progress of family/concerns to the named caseload holder.
