Job summary

The purpose of this post is to manage the one of the primary care based mental health locality teams prior to and during the transformation process, which offer access of mental health clients to physical health care assessment, specialist physical health support and relevant non statutory organisations, being responsible for the operation and development of the service, including management of staff and business planning.

These services will provide specialist support to primary care so that more people with complex mental health problems, including co-morbidities, can be managed in primary care, rather than being seen in secondary care services.

This would include working with people who are difficult for primary care teams to manage, but do not meet thresholds for or find it difficult to access, specialist secondary care MH services and are not appropriate for IAPT services

Main duties of the job

To be responsible for all aspects of the running and management of one of the six PCN locality teams in either Camden or Islington, including the staff rota and ensuring an equitable distribution of duties and responsibilities.

To manage the development and the implementation of individualised and up to date support plans ensuring changes in support plans are evaluated with the service users.

To research and organise activities for groups and individuals which may focus on educational, leisure, vocational or therapeutic themes, and which involves both mental health specific and mainstream services.

To take a lead role in the referral process of new service users, which will include community visits, liaison with local GP practices, other health, social care and housing providers and to ensure all documentation is completed.

To provide professional supervision and the formal appraisal of identified staff ensuring training plans contribute to the overall needs of the service and are in line with organisational goals.

To ensure that appropriate strategies are in place for staff to engage service users effectively with the agreed support plan and access to appropriate services is provided on a regular and consistent basis.
