Are you a dedicated and positive Health Improvement Practitioner looking to join an organisation focused on professional growth and development? You will have the chance to contribute to the delivery of the stop smoking, physical activity interventions and wider health education interventions.

An exciting opportunity has arisen to join our Award-Winning Wellbeing Service team. The role will specialise in reducing and preventing Obesity and Type 2 Diabetes through delivering groups (face to face and online) and 1-2-1 in the community.

As a Health Improvement Practitioner, you will work 22.5 hours per week where your working hours will be 9 to 5. The days worked can either be Monday to Wednesday or Tuesday to Friday but occassional weekend work is to be expected. This will include some ad hoc community events within the local area.

Main duties of the job

As a Health Improvement Practitioner within HCRG Care Group youll join a highly skilled team who are focused on service excellence.

Your days will be rewarding and varied and include:

  • Be a key part of our Adult Weight and Diabetes team delivering behaviour change interventions to adult residents in Bath & N E Somerset. This will include planning courses, identifying need, organising venues and contacting attendees. Calling referrals and triaging them to the most appropriate service and maintaining clinical records.
  • Be part of a wider team and as required, assist, promote and deliver wider Wellbeing services e.g., stopping smoking, childrens healthy weight, physical activity, mental wellbeing and the Community Wellbeing Hub.
  • Support, develop, facilitate, and evaluate services and training courses for general public and a range of health promoters in partnership with colleagues.
  • Raise awareness of the Wellbeing Service including the Community Wellbeing Hub and disseminate information to relevant organisations e.g., Council, Charities and service users.
