Job summary

An exciting opportunity has arisen to join the Continuing Health Care Team in Surrey.

The successful applicant will be responsible for the oversight of the Acute Liaison Practitioners who are based within the acute hospitals in Surrey overseeing the Discharge to Assess Pathway. In addition they will have the overall management of the Transition team ensuring the smooth transition of care between children’s and Adult Continuing care, working closely with social care colleagues.

Main duties of the job

To be responsible for the line management of the CHC Acute Practitioners and the Transition Service Lead

To ensure that the clinical team provide the level of input and support required to enable the Acute practitioners deliver a robust service that includes the processing of the Discharge to assess process and support to Acute hospitals in relation to the discharge planning of complex care patients. To ensure clinical staff participate, where relevant, in eligibility and disputes panels, following published national guidance and aligning with the NHS National Framework for CHC and FNC.

To be directly responsible for the leading and managing the Responsible commissioner process, following published national guidance, and aligning to the Who Pays? – Determining which NHS commissioner is responsible for making a payment to a provider guidance of August 2020.

To be directly responsible for line management for the Specialist Lead of the CHC Transitions team ensuring that robust transition processes are in place and inter-agency working follows the national guidance aligning to the national framework.

The Continuing Healthcare Team is part of Surrey Heartlands Integrated Care Board and offers Hybrid working Monday to Friday between 9am and 5pm.
