Sutton PCNsare looking to employ 2 Occupational Therapists to support the Sutton PrimaryCare Network team in the delivery of patient services, working as part of thePCN multidisciplinary team, delivering care within their scope of practice tothe entitled patient population in a community ward. Main duties of the job
Sutton PCNs would like enthusiastic and highly motivated Occupational Therapists with Community or Hospital background experience to work at the PCN Community ward. The role requires excellent communication skills as you will be undertaking complex care planning assessments and developing and delivering programmes of care for patients and carers. The successful candidate will be committed to working closely with other health professional including doctor, nurse practitioners, HCAs and ward coordinators to provide high quality care to patients with a wide range of medical conditions in the Community Ward. Flexible working will be required to meet the needs of the patients group and the service as it continues to develop. You will be joining an enthusiastic and motivated team who are friendly and approachable. We are very patient focused and staff development is high on our agenda. As part of our team we want to develop all staff to the highest standard.

About us
Sutton has a population of approximately 200000residents registered to 23 practices and there are currently 4 Primary CareNetworks (PCNs); Carshalton, Cheam & South Sutton, Central Sutton andWallington PCN; each serving a population of approximately 50000 patients. Primary care networks (PCNs) form a key buildingblock of the NHS long-term plan. Bringing general practices together to work atscale has been a policy priority for some years for a range of reasons,including improving the ability of practices to recruit and retain staff; tomanage financial and estates pressures; to provide a wider range of services topatients and to more easily integrate with the wider health and caresystem. Key Responsibilities:
Assess, plan, implement, and evaluate treatment plans, with an aim to increase patients productivity and self-care.
To be accountable and responsible for devising, developing, delivering and updating individualised rehabilitation
To undertake assessments, and using clinical reasoning skills, knowledge of evidence-based practice and experience, determine appropriate treatment/support plans.
Work with patients through a shared-decision making approach to plan realistic, outcomes-focused goals.
Undertake both verbal and non-verbal communication methods to address the needs of patients that have communication difficulties.
Work in partnership with multi-disciplinary team colleagues, physiotherapists and social workers, alongside the patients’ families, carers, and employers in treatment planning to aid rehabilitation.
Where appropriate, support the development of discharge and contingency plans with relevant professionals to arrange on-going care in residential, care home, hospital, and community settings.
Periodically review, evaluate and change rehabilitation programmes to rebuild lost skills and restore confidence.
As required, advise on home and workplace environmental alterations, such as adjustments for wheelchair access, technological needs, and ergonomic support
Advise patients, and their families or carers, on specialist equipment and organisations that can help with daily activities.
