Health Care Assistant – Band 3

We are an integrated care organisation and our community and hospital services are one.

We are a 24 hour service and this role involves rotational shifts between 8:30am-5:00pm as well as 2:00pm-10:00pm

Main duties of the job

Health Care Assistants are important and valued members of our District Nursing teams and this is an excellent opportunity for a Health Care Assistant to join our team. You need to be self-motivated, an excellent communicator and team player with an NVQ level 3 in care (or equivalent). In return we offer you supervision, support, competency assessment and training in order to develop your skills.

    MAIN DUTIES*Participate in the assessment of care needs and the implementation of programmes of care in partnership with patients/carers in their own homes*Identify changes in patients conditions and, within their capabilities, recognise where significant changes in the care plan are required and report these back to the senior nurse on duty in a prompt and timely manner.*Liaise effectively with other nursing and multi professional team members within the scope of the Health care Assistant role and report back to the nurse in charge.

    Be competent in additional clinical skills and knowledge areas, for example:*Blood pressure monitoring*Blood sugar monitoring*Administration of medicines via gastrostomy plus setting up of gastrostomy feeds*Administration of oral medication.*Simple sterile dressings*Best infection control practice

    Be willing to quickly undertake further training/supervision in other clinical skills, knowledge areas for example:*Continence re-assessment*Venepuncture*Injection technique*Diabetes care*Medication review*Chronic disease management*Pressure area care*Bowel care*Palliative care
