We are excited to offer a new position within the older adult and frailty division. This will be a rotational post working across the dementia care pathway, it provides an opportunity to work across both the dementia home treatment team and the inpatient ward at Witham Court, Lincoln.

  • The post holder will be responsible for the assessment and risk assessment, planning and implementation of evidence-based care to service users with a known or suspected diagnosis of dementia. This will include co-ordination of care needs, and the assessment, planning and implementation of evidence-based care to a defined group of service users within the service catchment area under the guidance of senior staff. The post holder will provide professional advice and support to other agencies and members of the multi-disciplinary team.
  • The primary purpose of the DHTT is to provide a timely intensive home treatment service to Older Adults with dementia who are presenting with behavioural and emotional disturbance to a degree where the risk they pose indicates that an inpatient admission may otherwise be required. This is a step up/step down service.
  • Patterns of working across DHTT are 08:00-20:00, 7 days a week on a shift pattern. Shifts are usually 11:15 hours with a 45-minute break working for 3 days one week and 4 days the following with every other weekend off.
  • Main duties of the job

This will include co-ordination of care needs, and the assessment, planning and implementation of evidence based care to a defined group of service users within the service catchment area under the guidance of senior staff. The post holder will provide professional advice and support to other agencies and members of the multi-disciplinary team.
  • The primary purpose of the service is to provide a timely intensive home treatment service to Older Adults with dementia who are presenting with behavioural and emotional disturbance to a degree where the risk they pose indicates that an inpatient admission may otherwise be required.
  • The post holder will assess care needs and develop, implement and evaluate programmes of care for those who are experiencing acute episodes within agreed limitations.
