    Main duties of the job

    Swansea Bay UHB Urgent Primary Care Service is offering an exciting opportunity for highly motivated and experienced Pharmacists to support the Urgent Primary Care (UPC) service. 

    You will be responsible for improving the quality and effectiveness of medicines management in line with Prudent Healthcare principles and for developing, delivering and monitoring medicines management services within UPC. 

    You will deliver pharmaceutical care to a range of patients including those who may be frail and vulnerable and have complex needs, via triage or face to face appointments.

    You will provide expert pharmaceutical advice, education and training to prescribers and other healthcare staff working within UPC, developing close working relationships with a variety of health and social care professionals to maximise seamless care, ensuring the best outcomes for patients. 

    You will develop and extend the role whilst ensuring scope of practice is in line with competencies as defined within individual personal boundaries of clinical practice.

    The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

  • Independent prescriber status
  • Desirable criteria

  • Registered with RPS
  • Working knowledge of prescribing software (CASPA) and GP in-practice clinical systems
