The Continuing Healthcare Team are looking for an enthusiastic & motivated individual to join the team in a new & innovative role.
The team are recruiting for an Assistant Practitioner/CHC administrator at Band 4 level.
Applicants should be educated to NVQ 3 level in a relevant subject or hold an equivalent level of qualification or have equivalent experience.
We are looking for an individual with an understanding of the CHC process who would like to visit individuals in their own & care homes to collate relevant information to support the CHC assessment process.
The role includes liaising with family members & other members of the wider multi-disciplinary team during the preparation for the assessment.
We are looking for an individual who is able to work autonomously with support & as part of a wider team. The individual will work closely with the administrative & the specialist assessor team.
The individual will need to be able to work to a time frame as there will be a period of time from allocation to assessment when a draft DST will need to be populated, all relevant information collated & the draft DST fully prepared for assessment.
The role will also include day to day administrative duties as required & capacity allows.
The CHC team are a small team & very welcoming to new staff & training and support will be given for this new role.

Main duties of the job
The main dutiesof this role will be the population of Continuing Health Care Decision SupportTools and Review forms, the collation of relevant clinical information tosupport CHC assessments using electronic systems and care home and domiciliarycare records and the preparation of the DST / review form which will enable thespecialist assessor to complete the assessment. This role will include visitingcare homes and individuals in the community to obtain the relevant clinicalinformation to identify health care needs as described in the CHC NationalFramework. Communication with other professionals involved in the individualscare will also support with information gathering for the DST / review. Therole will also include communication with families and providers re theassessment and liaising with the specialist assessor.
The AP will beable to work flexibly with the expectation that this will include some officebased work and some home working.
In addition,on an as needed and as capacity allows basis the AP will support with generaladministrative duties i.e answering telephones and directing calls, mailmanagement, management of the Hub emails, maintaining and uploading to electronicsystems and other day to day administrative roles.

8.1 Highly motivated and enthusiastic individual to provide and coordinate administrative and secretarial services to include sourcing and collating records, populating national tools for CHC, the preparation of agendas and minutes, taking appropriate follow-up action as required
8.2 The post holder will be required to review the case notes of clients who are requesting NHS Continuing Health Care funding or require review. This will involve travel to care homes and possibly other locations. They will be identifying evidence of health care needs following the guidelines as described in the National Framework for NHS Continuing Healthcare and NHS-Funded Nursing Care using various computer systems and liaising with other professionals
8.3 To use the information collated to support the CHC contribute to the clinical review process, to include confirming that a review had been completed and that the needs were the same and being met. To flag to the team if a more in depth review is required or that the care needs had changed
8.4 The post holder will work under the supervision of Clinicians who will use this evidence to form the basis of their CHC assessment and will assist with typing final document as required
8.5 The post holder will work closely with the admin team to maintain the general running of the department
8.6 Supporting the team with the management of projects, gathering information and undertaking enquires as and when is necessary for the head of department, teams and the department

Provide specialist secretarial/administration support and advice to the teams and/or Sector by:
Making and receiving telephone calls for the department as necessary and using initiative to deal with phone calls and messages
Sorting and prioritising all incoming mail and e-mail, distributing as appropriate
Managing the electronic diary for the assessors as required, including arranging and changing appointments, prioritising these as appropriate
Undertaking administrative duties such as photocopying, faxing and mail-out distribution Ensuring all urgent and/or confidential communications are received and distributed from/to relevant parties in a timely manner
Organising and planning events as necessary including gathering and distributing supporting information/material
Supporting teams in project management and participating in department events
Inputting, monitoring and checking data, required for finite and ongoing projects within the teams
Working with all team members in the collection of information for performance reporting on relevant team projects
Acting as a point of contact for teams, dealing and responding effectively with complex queries from stakeholders and passing on relevant information to appropriate team members sensitively and autonomously
Supplying the relevant information required for financial management, supporting the head of department and teams by checking and sending invoices for payment. Ordering stationery as required
