We have an exciting opportunity for a new role of First Contact Practitioner – Dietitian with the York Integrated Care Team (YICT )

The YICT was initiated by Priory Medical Group who designed, developed and implemented the care model which was born from the views of the local community who said they wanted fast access to care and support and to only have to tell their story once.

    You will also be joining a growing team of over 30 Dietitians in the York and Scarborough Teaching Hospitals NHS Foundation Trust. This is a forward thinking Nutrition and Dietetic department, which is valued and respected throughout the Care system and crosses many specialities and localities. 

    The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy.

    Main duties of the job

    The role will primarily focus on dietetic intervention, education and training in relation to frail adults and older people including those who require nutrition support and live with a care home setting covering patients attached to Priory Medical Group, Haxby Group (York) and York medical group. 

    We will support you within a friendly dietetic team. You will have opportunity to work alongside a network of well-established Multidisciplinary Team within the York Integrated Care Team. We will provide training to develop your skills and knowledge. 

    Patients receive dietetic input in a variety of settings including Care homes, GP practices, virtual and telephone appointments. 

    The department places a strong emphasis on clinical and peer support and supervision, and has a well established programme facilitating the training of placement A, B, and C dietetic students. We are also involved in delivering a variety of rolling training programmes for other healthcare professionals across the Trust, relating to nutritional care. The successful candidate will be involved in these important aspects of learning and development.

    Essential criteria

  • Highly proficient organisational and prioritisation skills
  • Evidence of ability to lead engagement activities with others and understanding of the importance of good communication when implementing change
  • Ability to work within multidisciplinary teams acting as an advocate for dietetics and authority in relevant specialist area
  • Excellent communication skills both written and verbal
  • Desirable criteria

  • Evidence of enhanced communication skills and the ability to use different communication methods and styles, relevant to specialty
  • Evidence of significant personal development in relevant specialist area
  • Experience & Knowledge Required

    Essential criteria

  • Experience in specialist role, including the management of a range of specialist conditions, with substantial evidence of additional knowledge acquired through formal and informal clinical supervision
  • Evidence of ability to lead and act as an authority on the management of complex clinical presentations in specialist area
  • Substantial experience of assessment and provision of specialist equipment
  • Significant experience of delivering clinical treatment within specialist area
  • In-depth understanding of current issues in health care Standards of Professional Practice
  • Desirable criteria

  • Highly experienced in the identification and use of outcome measures to evaluate clinical treatment
  • Highly developed specialist clinical skills within relevant area
  • Significant experience of training/supporting others (including students) within specialist clinical area
