We are looking for an enthusiastic, committed individual with a relevant clinical registration (nursing, OT, social work) to join the Peterborough community mental health team for older adults. We are a friendly and supportive multidisciplinary team embedded within an exciting Integrated Care model. We provide patient focused, evidence-based mental health interventions and assessment for people aged 65 and above, with a range of Functional and Organic mental health illness. You will manage a caseload of service users, delivering a range of clinical skills and therapeutic and recovery-based interventions.

The ideal candidate will have a strong team work ethic and sense of accountability, accompanied by an ability to work under pressure at times. We support agile working and promote regular supervision and continued professional development. You will mainly be covering Peterborough City and on occasion the surrounding villages. Therefore, successful applicants will need to be able to travel independently and in a timely manner.

Main duties of the job

Mental Health practitioners in Neighbourhood Teams will provide assessment and interventions for people with significant mental health conditions including dementia and functional mental health illnesses.

 Offer training, advice and guidance to colleagues in the neighbourhood team on common mental health conditions.

Participate in Neighbourhood Team MDT meetings and ensure key information is recorded in shared care plans.

Work closely with the mental health integrated care team in order to provide an excellent and holistic mental health service.
