An opportunity has arisen for 2 enthusiastic individuals to join the capacity team as Band 4 Patient Flow co-ordinators on a permanent basis in a new and exciting role.

The patient flow co-ordinators will support with the timely identification, planning and discharge of inpatients across our acute site.

The Capacity team are a well established team of senior nurses supported by the wider Multidisciplinary Team (MDT) with the safe effective placement and discharge of inpatients across the acute site. 

Main duties of the job

You will be able to provide consistent patient- centered focus throughout the pathway of patient care by liaising with a variety of members of the MDT to support safe and effective discharges. 

You will lead and pro actively advocate for the patient and safe discharges at clinical board rounds. 

You will support ward staff to facilitate the appropriate discharge of patients in a timely manner.

You will develop professional relationships with health and social care providers (both internal and external) to plan and deliver discharge plans with the support of the capacity team. 

You will attend regular capacity meetings throughout the day, and with support of the capacity team will pro actively escalate and resolve any delays or potential concerns with planned discharges.

    Detailed job description and main responsibilities

  • Liaison with health and social care providers to plan and deliver discharge plans
  • Lead board rounds in clinical ward areas and actively support discharge.
  • Work alongside the nurses, and any/all other professionals, patient and family in relation to discharge.
  • Checking the patients have the appropriate equipment in place on discharge and inform the District Nurses.
  • Support staff to facilitate discharge patients.
  • Ensure that up to date, relevant and accurate information is proactively and accurately maintained on the database for reporting purposes.
  • Support medical wards with safely discharging patients timely and liaising with discharge coordinators, ward coordinators, community teams and the bed management team.

    Experience

    Essential criteria

  • Relevant experience at a band 3 or above in an acute setting
  • Experience of working in an acute care environment
  • Previous experience of patient discharges
  • Previous experience of working in a multidisciplinary team
  • Previous experience of leading clinical board rounds
  • Previous experience/ confident in professionally challenging
  • Experience of providing direct care to individuals
  • Qualifications

    Essential criteria

  • Educated to NVQ level 3 or equivalent
  • BLS certificate or equivalent
  • Manual Handling
  • Computer literate
  • Articulate and literate
  • Desirable criteria
