We have a vacancy for a motivated and enthusiastic Children’s Nurse (RN Child) who would like the opportunity to develop their expertise as a Children and Young People’s Community Nurse within our friendly team in Chichester. This is a developmental role to support a Band 5 to progress to a Band 6 post . The expectation is that the successful candidate will complete a Safeguarding and Physical Assessment module. We are looking for a skilled clinical nurse who is able to work autonomously in the community as well as part of a wider team. Currently the team works seven days per week.

The Children’s Community Nurse (CCN) will contribute to the development of the local Children’s Community Nursing Team and countywide CCNS together with the Team Lead and senior nurses, helping to ensure that policy and practice is developed and delivered in line with Trust and national best practice standards.

Main duties of the job

The Chichester team work in partnership with The Sussex Snowdrop Trust by providing care to those children with a life threatening or life-limiting illness. The service provides community nursing for children and young people with long term conditions and complex needs, and palliative care, including end of life care. The children can present with acute illness in addition to requiring skilled nursing intervention to manage highly complex health and developmental needs throughout childhood and during their transition to adult services. The CYPCNS works closely with social care, education and third sector organisations to implement the statutory requirements of the Children and Families Act 2014, and we are actively involved with partners in developing children and young people’s urgent care pathways.

Professional, operational and strategic leadership is provided through the Head of Children and Young People’s Community Nursing, with additional leadership for quality and standards provided through the Children and Young People’s Community Nursing Matrons.

SCFT is the lead provider of Children and Young People’s Community Nursing Services (CYPCNS) across Sussex. The CYPCNS has four Community Nursing teams in Crawley, Lancing, Chichester, and Brighton and Hove, and a Specialist Children’s Continence team.

Detailed job description and main responsibilities
Manage the delivery of quality nursing care, support, information and advice,
for a delegated CCNT caseload, helping to ensure that appropriate resources
are available to assist in providing this care.
Support and educate staff offering clinical specialist knowledge and skills for
the delivery of nursing care for children in the CCNT caseload.
Maintain excellent communication channels with service stakeholders.
Engaging in cross boundary partnership working including the child and
family/carer, to help develop seamless care provision. This includes working
with health care, social care and education as well as relevant charities
including the local hospices.
Help develop, implement and review clinical protocols and standard operating
procedures which assist in delivering safe care and best nursing practice.
Develop clinical skills and knowledge within the team, ensuring the relevant
clinical skills required for management of safe nursing care are maintained.
Provide teaching regarding the use of equipment and treatment regimes in the
community for children, families/carers and staff involved.
Ensure concise and accurate documentation of care interventions, including
telephone advice and support are documented and recorded.
Act as a resource and role model of children’s community nursing specialist
practice to all other professionals and work colleagues.
Attend essential and relevant clinical practice updates in tertiary centres and
special interest groups both within the Trust, locally and nationally to help
develop and contribute towards establishment of clinical practice standards.
Staff may be required to work anywhere in West Sussex to meet the needs of the commissioned service

Qualifications
Essential criteria

  • Registered Child
  • Current registration with NMC
  • Desirable criteria

  • Module in mentorship, ENB 998 or equivalent
  • Children’s Community Nursing qualification
  • Experience
    Essential criteria

  • Minimum of 1 year experience of children’s nursing post registration (RNC)
  • Experience working with children who have complex health needs and disability
  • Experience of working in partnership with other agencies
  • Relevant post registration experience in child health care
  • Desirable criteria

  • Experience of working with children at end of life
  • Experience of working in a community setting

    Essential criteria

  • Ability to assess health care needs for children aged between 0-19 years
  • Ability to assess health care needs within a community setting
  • Ability to recognise and take appropriate action to safeguard children, young people and vulnerable adults
  • Broad general knowledge of the developing child
  • Knowledge of relevant legislation and national guidelines
  • Working knowledge and evidence of implementation of holistic assessments and interventions relevant to children and young people
  • Ability to organise and respond efficiently to complex information
  • Ability to prioritise own workload and meet deadlines
  • Ability to pass on skills/knowledge to others within both formal and informal environments
  • Desirable criteria

  • Ability to communicate effectively complex and sensitive information both written and verbally

  • Holds full driving license and has access to a car to meet the travel needs of the role
  • Ability to work flexibly on a rota system within the team including weekends.
