An opportunity has arisen for a Community Phlebotomist to join Crawley Care Collaborative PCN. The successful candidate will work as part of a multidisciplinary team and undertake venepuncture, under the supervision and guidance of qualified staff, to patients living in their own homes. They will also have administrative duties including phoning patients, data input, and sending information to patients and other healthcare professionals.

Main duties of the job

Crawley Care Collaborative are part of a wider Primary Care Network (PCN) which is a multidisciplinary network, combining community nursing and proactive care teams with other professionals. PCNs are the core element of a wider system responsible for coordinating care delivery to patients on the caseload. Care is organised around individuals’ identified needs through collaborative working by service providers.

The post holder will need to be an experienced phlebotomist with excellent communication skills.

    Key responsibilities
    a) To assist and support colleagues in the delivery of a phlebotomy service to individuals within the community setting.
    b) To deliver holistic care when undertaking interventions to include pressure ulcer prevention advice and care, dementia awareness, observing nutrition and hydration status, falls risks, pain awareness and applying infection control standards.
