The Tywi Taf Cluster is one of the seven Primary Care Clusters in Hywel Dda University Health Board and is located in Carmarthenshire, which is mainly an agricultural county, apart from the south eastern region which includes Llanelli and towns in the Amman and Gwendraeth Valleys. The Cluster comprises of 8 General Practices. In addition to the 8 Practices, there are 2 branch surgeries. We are an innovative Cluster who are always looking at new and novel ways to improve patient care in their communities.

Main duties of the job
For this Cluster Pharmacist role you will be responsible for providing prescribing advice to GPs and other health professionals and undertaking medication reviews in GP practices and care homes with the aim of reducing risk, improving quality and promoting cost effective prescribing.
You will liaise with GPs, nurses, practice staff, carers, patients and community pharmacies and work closely with the GP Practice and cluster based teams.
As an Advanced Pharmacist, you will have an ability to develop effective working relationships, to challenge yourself and others, and to prioritise your own work.
You will need to be reliable, efficient and responsive with the ability to demonstrate a flexible approach to the needs of this challenging and growing service.
You will have excellent communication skills with a demonstrated ability to work autonomously and collaboratively to ensure patients with complex pharmaceutical needs across the cluster are reviewed effectively, minimising the need for GP intervention.
