A vacancy has arisen at Berwick Infirmary Older Persons Rehabilitation Ward for a dynamic, highly motivated and competent individual to join our team. We are a 16 bedded innovative and forward thinking older person rehabilitation ward; supporting a variety of patients who require ongoing medical care and rehabilitation following acute illness e.g. medical, orthopaedic, dementia and palliative care.

We provide excellent care to a wide range of patients with the emphasis on advanced discharge planning and working closely with Community and our Ward Multi- Disciplinary Teams.

The successful candidate will have excellent communication skills and a commitment to ensuring safe and high quality care is delivered to our patients, as well as being a role model and supporting our junior workforce.

We support the ongoing development of B5s to become competent and autonomous in their role within Community Hospitals

Main duties of the job

The successful post holder must be able to role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the Trust. 

This is a rotational post and the successful candidate will be expected to work night shift as service needs require.

Main duties of this role include;

To provide nursing care to a caseload of patients and to provide verbal and written handovers to nursing colleagues and the multi disciplinary team

When responsible for the ward/department to ensure that individualised programmes of care are formulated and carried out fully for each patient on the ward/department (up to 16 patients).
