We have an exciting opportunity for a motivated clinician to be an active member of a dynamic team consisting of clinicians and coordinators and administrators in the Responsive services central coordinator hub.

The RS central area coordinator hub ensures that all referrals are triaged and appropriate clinical decisions are made and accurate sign posting is offered to those who queries can be dealt with over the telephone.

The Senior Clinical Assessor will have the knowledge and capability to refer the patient onwards to the appropriate team or service as part of the ongoing ‘Flow’ through complex systems. The senior clinician will support and oversee the running of the hub on a daily basis. The hub is the heart of the wider Responsive Services Teams and supports the flow of patients through the services to maximize capacity and support the wider teams.

This is a role for an individual who would like to develop their leadership and management skills and clinical assessment skills in a different challenging environment. They may also be the opportunity to visit patients at homes when appropriate

Successful applicant would need to be proficient IT systems, predominantly Excel and Word, and possess excellent communication and interpersonal skills. They would also need to demonstrate initiative and have experience of autonomous working.

Main duties of the job

  • Is required to work autonomously reaching clinically appropriate decisions and use their knowledge and experience. Support and assistance may be requested entirely at the discretion of the individual. Is ultimately responsible and therefore accountable for their decisions made relating to patient care requirements.
  • The RS Area Wide Service Co Ordination (THE HUB ) Senior Clinical Assessor will ensure that the RS Area Wide Service (THE HUB ). The Senior Clinical Assessor will respond in a timely fashion in accordance with quality standards set, to callers needs with immediate life threatening calls being transferred to the appropriate service.
  • The Senior Clinical Assessor will share specialist knowledge with colleagues involved in the delivery of ongoing training, education and support of colleague
  • Provide clinical support via the telephone on care of the deteriorating patient and advice to unregistered staff as required.
  • To provide supervision and support to staff within the team through a supervision pathway. To support staff with the maintenance of training and competencies
  • Alongside the Operational Team Lead, take responsibility for staffing levels on day to day basis, authorising bank shifts and using the Health roster system for rota management.

    Detailed job description and main responsibilities
    The role is currently based at Crawley Hospital and is working alongside other members of the Multi-disciplinary team including physiotherapists, occupational therapists, and Health care assistants and liaising with colleagues in the community and the acute sector.

    The service runs 8am to 8pm and 7 days a week so all applicants would need to work shifts between these hours and on weekends on a rota.
